Malnutrition is a major health problem throughout the world and contributes to at least one third of all children deaths worldwide. Hypoglycemia basic metabolic complication in pediatric patients which can be easily prevented by proper care management. Objective this study was determine frequency hypoglycemia sever acute Malnurished (SAM). The conducted nutritional division Nangarhar University Teaching Hospital department. For 8 months, were subjected for measurement blood glucose level oxidase method .Hypoglycemia labeled according value mentioned operational definition. included total 252 with SAM. 137 (54.4%) Males 115 (45.6%) females no significant difference number gender an average age (Mean± SD) 16.20±11.74 (6 months 60 months). Out 54.80% (138) hypoglycemic, 85.70% (216) suffering from Marasmus. 38.9% children’s caregivers illiterate 86.5% poor middle-level socioeconomic status. 12.7%( 32) SAM dying, most patient province.
 Sever malnutrition (SAM) constitutes important cause mortality morbidity children, marasmus more common than kwashiorkor children; early diagnosis treatment prevent decrease significantly, education, socioecnomic status are prevention their complications.
